Request a Premium Indication
To request a premium indication specific to your specialty, you may:
- Complete the appropriate Premium Indication Request Form online and submit it via email.
- Complete the appropriate Premium Indication Request Form online, print it, and then either mail or fax the completed form to our office.
- Call us directly and request the appropriate Premium Indication Request Form for your specialty at (800)548-4479.

Report a Claim
In the event an incident has occurred that may lead to a claim, a letter of legal representation on behalf of a patient has been received or if you have been served a writ of summons or notice of suit, contact our office immediately. We will provide assistance in reporting any claims to your insurance carrier and provide guidance throughout the claims process.
